Why combine testosterone and hCG?
Testosterone from outside the body tells the pituitary to stop sending LH and FSH, and the testes slow down: sperm production falls. Adding hCG, which mimics LH, keeps the testes stimulated while testosterone does the work of raising the blood level.
Who is testosterone with hCG for?
Men who need TRT and care about fertility or testicular function. Men focused only on fertility may do better on hCG or enclomiphene without testosterone.
- Confirmed low testosterone
- Fertility matters to you, now or later
- Already on TRT and wanting to protect fertility
- Comfortable with injections
What the combination does not change
Adding hCG does not remove the monitoring testosterone needs.
Hematocrit. Still monitored; a hematocrit above 54% means the dose is paused or reduced.
Estradiol. Some testosterone converts to estradiol, and it is checked on follow-up labs.
Fertility is supported, not guaranteed. If conception is the near-term goal, your provider may suggest stopping testosterone altogether.
What the combination is for
Testosterone level. Testosterone corrects confirmed low testosterone.
Testicular function. hCG keeps the testes stimulated while on TRT.
Fertility support. hCG supports sperm production that testosterone alone suppresses.
One monitored plan. Hematocrit, PSA, estradiol and testosterone are tracked together.
Starting testosterone with hCG
Diagnosis, then a combined plan, then labs.
- Evaluation. Symptoms, fertility goals and any current TRT.
- Morning labs. Two early-morning testosterone tests plus LH, FSH, estradiol, PSA and a CBC.
- Combined plan. Testosterone and hCG, with how and when each is used explained.
- Follow-up. Labs after starting, then regular monitoring.
Common questions
Testosterone + hCG: frequently asked questions
Why add hCG to testosterone?
Testosterone switches off LH, so the testes slow down and sperm production falls. hCG mimics LH and keeps them stimulated.
Does testosterone with hCG guarantee fertility?
No. hCG supports sperm production, but if conception is the near-term goal your provider may recommend a plan without testosterone.
Is hCG FDA-approved?
Yes, hCG is an FDA-approved prescription drug; using it alongside TRT is off-label.
Can I add hCG if I already take testosterone elsewhere?
Your provider reviews your current treatment and labs before recommending a change.
