What is testosterone cypionate?

Testosterone cypionate is a long-acting injectable form of testosterone, and one of the most common ways testosterone replacement therapy (TRT) is given. It replaces testosterone directly, so the body's own signal to the testes (LH and FSH) falls, and sperm production falls with it. That tradeoff is why fertility plans are discussed at the first visit, and why some men are better served by enclomiphene or hCG.

Who is testosterone cypionate for?

Testosterone cypionate is for men whose symptoms and repeated morning labs confirm low testosterone. It is not a treatment for normal age-related tiredness in men whose testosterone is normal.

  • Men with symptoms such as low libido, fatigue or loss of muscle, and confirmed low morning testosterone
  • Men who are not planning children in the near term
  • Men willing to keep up regular lab monitoring
  • Not started in men with prostate cancer; a raised or rising PSA gets a urology evaluation first
Testosterone cypionate: risks and monitoring

TRT is a long-term treatment. Monitoring is what keeps it safe.

Hematocrit. Testosterone stimulates red blood cell production. A hematocrit above 54% means the dose is paused or reduced.

Fertility. Sperm production usually recovers after stopping testosterone, but recovery can take many months and is not guaranteed.

Heart. In the TRAVERSE trial of men at high cardiovascular risk, testosterone was noninferior to placebo for heart attack, stroke and cardiovascular death, but showed higher rates of atrial fibrillation, acute kidney injury and pulmonary embolism.

Injections or gel. Testosterone can also be given as a daily gel or cream. Your provider explains the differences in routine and in how levels rise and fall.

How testosterone cypionate care works here

Diagnosis before treatment. Low testosterone is confirmed with symptoms plus low morning blood tests, not a single reading.

A full baseline panel. The male hormone panel covers total and free testosterone, estradiol, LH and FSH, PSA and a CBC, which includes hematocrit.

Fertility discussed first. Testosterone suppresses sperm production. If children are in your plans, your provider will discuss alternatives that keep the testes working.

Monitoring that keeps it safe. Hematocrit, PSA and estradiol are rechecked after starting or changing a dose, then at least yearly once stable.

How testosterone cypionate treatment works in Scarsdale

The same steps apply whether you are seen at the clinic or by telehealth.

  1. Evaluation. A provider reviews your symptoms, history, medications and fertility plans.
  2. Assessment and morning labs. The Male or Female Hormone & Longevity Assessment ($349) includes bloodwork drawn at the Scarsdale clinic, a SECA body composition scan and a grip strength test. The Telehealth Hormone & Longevity Assessment ($249) reviews bloodwork by video.
  3. A plan that fits your labs. If you have hypogonadism, your provider explains testosterone and the alternatives. If you do not, you hear that too, along with what else may explain the symptoms.
  4. Follow-up labs. Labs and a check-in after starting, dose adjustments as needed, then regular monitoring for as long as you stay on treatment.
