Do you use hormone pellets?
No, and the reason is worth explaining because pellets are heavily marketed in northern Westchester. An implanted pellet delivers a fixed amount for months and cannot be adjusted or easily removed if levels overshoot — which in women in particular they frequently do. Most menopause societies advise against them for that reason. We use preparations whose dose can be changed when your labs or symptoms change, which is the entire point of monitoring.
What are the alternatives to pellets?
For women, transdermal creams, gels and patches, and oral micronized progesterone where it is indicated — all adjustable, all titrated against repeat bloodwork. For men, injections or transdermal preparations, chosen around lifestyle, fertility plans and how each behaves in the body. The common thread is that if a marker moves, the dose can move with it.
How do I know whether I need hormone therapy at all?
By testing before treating. For men that means two morning blood draws rather than one, alongside the pituitary hormones that reveal where the problem originates. For women it means oestrogen, progesterone and testosterone read together and timed to the cycle where one is still present. Both include thyroid, iron and metabolic markers, because those mimic hormone symptoms and are commonly missed.
