Hormone Replacement Therapy for Women at Protocol Health

What is hormone replacement therapy for women?

Hormone replacement therapy (HRT), also called menopause hormone therapy, replaces the estrogen, and usually progesterone, that the ovaries stop making around menopause. It is the most effective treatment for hot flashes and night sweats, and it also treats vaginal dryness and helps prevent bone loss. The Menopause Society's 2022 position statement concludes that for healthy women younger than 60 or within 10 years of menopause with bothersome symptoms, the benefits generally outweigh the risks. Starting later needs a more careful review.

When HRT is worth discussing

Perimenopause often starts in the early 40s, and one blood test cannot diagnose it, because estrogen and FSH swing from week to week. For women over 45 with typical symptoms, it is diagnosed from symptoms and cycle changes. Labs still matter, to rule out thyroid problems and anemia.

  • Hot flashes, night sweats or poor sleep
  • Irregular or heavier periods, mood changes or brain fog in your 40s
  • Vaginal dryness or painful sex
  • Early or surgical menopause
Is HRT safe? Timing, route and the 2025 label change

HRT is not one prescription. The dose, the route and whether progesterone is needed depend on your age, your symptoms, whether you have a uterus and your history.

Timing. Absolute risks were low in women aged 50 to 59 in long-term WHI follow-up. Starting for the first time after 60, or more than 10 years after the last period, needs a more careful review.

The boxed warning. In November 2025 the FDA began removing the broad boxed warnings on menopausal HRT, and it approved the first six revised labels in February 2026. It kept the endometrial cancer warning on systemic estrogen-alone products, because estrogen without progesterone can thicken the uterine lining.

Route. Estradiol can be given through the skin as a patch, gel or cream, or locally as vaginal estrogen. Your provider explains which route fits your symptoms and history.

Not risk-free. A label change does not make HRT right for everyone. Every plan starts with an individual risk review.

The building blocks of an HRT plan

Estradiol patch, gel or cream. Replaces estrogen through the skin for hot flashes, night sweats and bone protection.

Oral micronized progesterone. Protects the uterine lining. Anyone with a uterus taking systemic estrogen needs it.

Vaginal estrogen. Treats dryness, painful sex and recurrent urinary symptoms locally, with very low absorption.

Testosterone cream for women. Low-dose testosterone for low sexual desire after menopause, when other causes are addressed. Compounded, not FDA-approved for women.

How HRT works at our Scarsdale clinic

Four steps, in the clinic or by telehealth.

  1. Evaluation. A provider reviews your symptoms, cycle history, personal and family history and when your periods stopped.
  2. Labs. A baseline panel drawn at the clinic or near home rules out conditions that mimic perimenopause and makes follow-up safer.
  3. A plan. Estradiol, progesterone and, where appropriate, testosterone, in the route and dose that fit your history.
  4. Follow-up. Check-ins to adjust the plan as symptoms change.

Is hormone replacement therapy safe?

For healthy women younger than 60 or within 10 years of menopause with bothersome symptoms, the Menopause Society concludes the benefits generally outweigh the risks. It is still an individual decision made with a clinician.

Can a blood test tell me if I am in perimenopause?

Not reliably. Estrogen and FSH swing widely during perimenopause, so a single draw can land on a normal day. For women over 45 with typical symptoms, it is diagnosed from symptoms and cycle changes.

Do I need progesterone with estrogen?

If you have a uterus and take systemic estrogen, yes. Progesterone protects the uterine lining.

Are bioidentical or compounded hormones safer?

Compounded hormone preparations are not FDA-approved. Your provider explains which products are FDA-approved and which are compounded before you decide.

What does the hormone assessment cost?

The Female Hormone & Longevity Assessment is $349 at the clinic and includes bloodwork, a SECA body composition scan and a grip strength test. The Telehealth Hormone & Longevity Assessment is $249.
