How topical testosterone works

A gel or cream is rubbed into the skin once a day, and testosterone is absorbed into the bloodstream from there. Levels stay steadier day to day than with weekly injections, at the cost of a daily routine. Like any testosterone from outside the body, it lowers the brain's signal to the testes and suppresses sperm production.

Who chooses testosterone gel or cream?

Men with confirmed low testosterone who prefer a daily topical to injections, and who are not planning children in the near term.

  • Confirmed low morning testosterone with symptoms
  • A preference for no injections
  • Willing to apply it at the same time every day
  • Not planning children soon; fertility-preserving options exist
What to weigh before choosing a topical

The form changes the routine, not the core risks of testosterone therapy.

Fertility. Topical testosterone suppresses sperm production, just as injections do. Enclomiphene or hCG are the options for men planning a family.

Hematocrit and PSA. Both are monitored on any testosterone. A hematocrit above 54% means the dose is paused or reduced.

Daily habit. Missed days mean lower levels. Men who struggle with a daily routine often do better on injections.

Gel or cream versus injections

No needles. A daily application replaces a weekly or twice-weekly injection.

Steadier levels. Daily dosing avoids the peak-and-trough pattern some men notice between injections.

Same diagnosis first. Low testosterone is confirmed with symptoms and two early-morning blood tests before any form is prescribed.

Same monitoring. Hematocrit, PSA, estradiol and testosterone are checked after starting and at least yearly.
