Sermorelin Before and After: What the Trials Measured

Sermorelin before and after is sold with photographs, and the published trials are not built from them. The adult trials measured growth hormone, IGF-1, lean mass, strength, skin thickness, sleep and mood instead. IGF-1 rose by week two and drifted back toward baseline by week 16. In one small trial lean mass, well-being and libido moved in men only, while preliminary results from a University of Washington sermorelin acetate program reported women not on estrogen replacement responding much as men did. No adult study found better sleep. Sermorelin prescribed today is compounded and not FDA-approved.

Sermorelin Before and After: What the Trials Measured

Quick Answer

What Sermorelin Before and After Means in the Research

Sermorelin before and after, in the research record, means measured changes in hormones, body composition and symptom scores rather than photographs. The published studies tracked growth hormone output, IGF-1, lean mass, strength, skin thickness, sleep quality, mood and libido. They never tracked appearance. We could not find a published trial of sermorelin or its close analogs that reports a before-and-after photograph of an adult.

Two of those measured findings run against the way sermorelin before and after is usually sold online. No adult study found better sleep: a 20-week placebo-controlled trial of a modified analog found sleep quality unchanged in both sexes, and a five-month placebo-controlled trial of sermorelin acetate found self-rated sleep slightly worse on treatment. In the 20-week trial, IGF-1 also rose within two weeks and then drifted back toward baseline by week 16 while the nightly injections were still being given.

The honest version of sermorelin before and after is therefore short: a handful of markers that moved, a smaller handful of outcomes that moved in only one sex, and a list of risks that small studies were never large enough to rule out.

The Measured Timeline

What the Sermorelin Trials Measured, and When

The sermorelin trials measured growth hormone output, IGF-1, body composition, strength, skin thickness, sleep and mood, on schedules running six to twenty weeks. Four adult datasets make up the record: a two-week study and a six-week study in older men, a 20-week trial of a modified analog in 19 adults, and a University of Washington program of placebo-controlled sermorelin acetate trials in older adults, the largest of which enrolled 89. Together they are the measured sermorelin timeline.

The 20-Week Placebo-Controlled Trial

The 20-week placebo-controlled trial was single-blind, with a four-week nightly placebo run-in followed by 16 weeks of a growth hormone-releasing hormone analog in 19 adults aged 55 to 71. Ten women and nine men took part, and the trial administered 10 micrograms per kilogram nightly by subcutaneous injection (Khorram, Laughlin and Yen, Journal of Clinical Endocrinology and Metabolism, 1997, PMID 9141536). Growth hormone rose within ten minutes of each injection, and the response held across the study. IGF-1 and IGFBP-3 rose within two weeks, held for 12 weeks, and were drifting back toward baseline by week 16.

One honest caveat belongs here rather than in a footnote. The compound studied was [Nle27]GHRH(1-29), a modified analog of the first 29 amino acids of growth hormone-releasing hormone, not sermorelin acetate itself. It is close kin to sermorelin and still not a sermorelin trial; the University of Washington program further down is.

The Six-Week Nightly-Injection Trial

The six-week nightly-injection trial gave 11 healthy men aged 64 to 76 a nightly subcutaneous injection of sermorelin, unmodified GHRH(1-29), at home for six weeks. The trial administered 2 mg each night, and every man had a low IGF-1 at baseline (Vittone and colleagues, Metabolism, 1997, PMID 9005976). Night-time growth hormone release went up on three separate measures. IGF-1, IGFBP-3 and growth hormone binding protein did not move at all.

That second trial is the one the marketing pages leave out, because its body-composition result was flat. Weight, body mass index, waist-to-hip ratio and DEXA-measured muscle and fat were unchanged after six weeks, as were glucose, insulin, lipids and muscle histology. Two of six strength measures and one endurance measure improved. The authors wrote that the data suggest a single nightly dose is less effective than multiple daily doses at producing growth hormone and IGF-1 effects.

The Two-Week Twice-Daily Study

The two-week twice-daily study is the earliest of the four, and it measured hormones only. Ten men averaging 68 years old took sermorelin, GHRH(1-29), by injection twice daily for 14 days at two doses (Corpas and colleagues, Journal of Clinical Endocrinology and Metabolism, 1992, PMID 1379256). At the higher dose, 24-hour growth hormone output and IGF-1 rose to levels that no longer differed from a group of young men, with no change in fasting glucose or blood pressure. The two-week study shows an older pituitary can still answer the signal; it measured no body composition, strength or appearance.

The Six-Month Sermorelin Acetate Program

The six-month sermorelin acetate program at the University of Washington is the largest controlled sermorelin dataset in adults. The group randomized 89 healthy adults averaging 68 years old to nightly growth hormone-releasing hormone or placebo for six months, and reported better scores on several cognitive tests, independent of sex or estrogen status (Vitiello and colleagues, Neurobiology of Aging, 2006, PMID 16399214). That paper supports the cognition result only.

The group's earlier review names the drug as sermorelin acetate (Geref), one evening injection, and gives preliminary results from two of its trials, one five months long and one six months long in which women not on estrogen also followed an exercise program (Vitiello and colleagues, Dialogues in Clinical Neuroscience, 2001). Men roughly doubled 24-hour growth hormone output and raised IGF-1 about 40%; women not taking estrogen replacement raised IGF-1 about 30%, and women on oral estrogen less than 10%. Body fat fell about 5% in men and in women not on estrogen, with a matching rise in lean mass. In the five-month trial, self-rated sleep got slightly worse on treatment and did not change on placebo. These are preliminary review analyses, so the six-month sermorelin acetate program is strongest on cognition and only suggestive elsewhere.

Read that table as the real sermorelin before and after: a reliable hormonal signal, a thin set of outcomes that shifts with sex and estrogen status, and several things that simply did not move.

Men and Women

What Changed for Women and What Changed for Men

What changed for women and what changed for men depends on which study you read, and on whether the women were taking estrogen. In the 20-week analog trial, women and men both gained measurable skin thickness over 16 weeks, while lean body mass, insulin sensitivity, general well-being and libido improved in the men only.

Nocturnal growth hormone rose significantly in both sexes in that trial, yet the downstream outcomes did not follow in women. The authors wrote that the observations suggest anabolic effects favoring men more than women, and that further studies are needed to define the difference.

The University of Washington sermorelin acetate program points the other way for one group of women. Its preliminary results showed women not on estrogen replacement raising IGF-1 about 30%, against about 40% in men, and losing about 5% of body fat with a matching gain in lean mass (preliminary, and in one trial alongside exercise), much as the men did. Women taking oral estrogen had the strongest growth hormone response yet the smallest IGF-1 rise, under 10%, which the authors read as estrogen-induced growth hormone resistance.

Searches for sermorelin for females before and after, sermorelin before and after women and sermorelin before and after female deserve that nuance rather than a slogan. A fair answer is that sex and estrogen status both change the sermorelin response, the female data are small and partly preliminary, and none of it was measured with a camera.

What Photos Miss

Why a Sermorelin Before-and-After Photo Proves Nothing

A sermorelin before-and-after photo proves nothing on its own, because every element that would make it evidence is missing from it. Protocol Health publishes no before-and-after photographs for sermorelin, and the reasons are not squeamishness.

  • No control. Nobody starts a peptide in isolation. Training, diet and sleep usually change in the same month, and a photograph cannot separate them.
  • No measurement. The outcomes that actually moved in the trials were skin thickness by caliper, lean mass by DEXA and IGF-1 by assay. A camera reads none of them, and the outcomes a camera does suggest, weight and DEXA fat, were the ones that did not change.
  • Selection. Clinics publish the responders, never the denominator.
  • Timing and framing. Photographs are taken when someone is pleased with them. Lighting, posture, pump, hydration and time of day move an image further than 16 weeks of a GHRH analog moved lean mass in the trial.
  • Reversal inside the window. IGF-1 in the 20-week trial was already heading back toward baseline at week 16. A week-8 photograph and a week-16 photograph are not measuring the same hormonal state.

Searches for sermorelin before and after pictures and sermorelin reddit before and after are looking for exactly this kind of proof, and the proof does not exist in the published record. A sermorelin before-and-after photo is an anecdote with good lighting.

Risks

The Risks and Unknowns of Sermorelin

The risks and unknowns of sermorelin carry equal weight on this page, because the outcome evidence is thin enough that the unknowns are the larger half of the picture. The adult studies reported few adverse effects, and that is a statement about their size as much as about the drug.

  • Reported in the trials. The 20-week trial's only adverse effect was a temporary rise in blood lipids, which resolved before the study ended. The six-week trial reported no significant adverse effects in 11 men, and the University of Washington program reported injection-site redness or swelling only rarely.
  • Commonly reported in practice. Injection-site redness, itching or discomfort, flushing, headache and lightheadedness are the effects clinicians see most often. These are clinical reports, not trial endpoints.
  • IGF-1 as a safety parameter. Sermorelin works by raising growth hormone and, through it, IGF-1, and IGF-1 is itself a growth signal. Clinicians monitor IGF-1 as a safety parameter, alongside glucose, not only as proof the drug is working.
  • Glucose. Growth hormone opposes insulin. The two-week and six-week studies found no change in fasting glucose, and the 20-week trial found insulin sensitivity improved in men and unchanged in women, so the direction is not settled.
  • Sleep. Sleep is sold as a benefit, yet no adult study found better sleep, and the five-month trial in the same program found self-rated sleep slightly worse on treatment.
  • Not yet known. No adult study ran past six months, enrolled more than 89 people, or was powered to detect uncommon harm. Cancer risk, cardiovascular outcomes and effects beyond six months are unknown rather than reassuring.
  • Who should not take it. Active malignancy is the clearest contraindication clinicians apply, because growth hormone and IGF-1 are growth signals. Pregnancy, nursing and untreated pituitary disease belong in the same conversation.

Weighed honestly, the risks and unknowns of sermorelin are mostly unknowns, and a short list of mild reported effects is not the same thing as a long safety record.

Regulatory Status

Is Sermorelin FDA Approved? The Geref Record

Sermorelin is not FDA approved today, and the Geref record is why that sentence needs a date attached to it. Geref was the brand name of sermorelin acetate, and the primary regulatory record is specific about what it was approved to do.

Geref injection at 0.05 mg base per ampule was approved on December 28, 1990 under NDA 19-863, for evaluating whether the pituitary gland can secrete growth hormone, which is a diagnostic use. Geref injection at 0.5 and 1.0 mg base per vial was approved on September 26, 1997 under NDA 20-443, for treating idiopathic growth hormone deficiency in children with growth failure. EMD Serono notified the FDA in 2008 that both were being discontinued, and the agency withdrew approval of both applications effective June 18, 2009 (FDA determination, Federal Register, March 4, 2013).

Two things follow that the marketing pages tend to blur. Geref was never approved for adult body composition, energy, sleep or anti-aging, so sermorelin has no FDA-approved adult outcome indication to point back to. And in 2013 the FDA determined that Geref was not withdrawn for reasons of safety or effectiveness, which means the withdrawal was a commercial decision rather than a verdict against the molecule.

Every sermorelin prescribed in the United States today is therefore compounded. The FDA states plainly that compounded drugs are not FDA-approved and that it does not verify their safety, effectiveness or quality before they are marketed. That makes sermorelin an unapproved drug rather than an off-label one, and it makes the pharmacy behind it part of the answer to any sermorelin before and after question.

Our Approach

How Protocol Health Handles a Sermorelin Before and After Question

Protocol Health handles a sermorelin before and after question by starting with your labs and history rather than with a picture. We are a family-owned, physician-led clinic, and the first job is deciding whether a growth hormone-releasing peptide belongs in the conversation at all. Slow recovery, low energy and a changing waistline have many causes besides growth hormone.

If sermorelin is appropriate, we treat it as a monitored trial with pre-defined markers rather than a product with a promised outcome. That means baseline labs, a clinician-set plan, and follow-up where the question is whether anything measurable changed. Readers comparing options can look at what sermorelin is and how it works, sermorelin versus ipamorelin and sermorelin versus tesamorelin before deciding anything.

A consultation is the first step. Protocol Health sees patients in person only at 700 White Plains Rd #30 in Scarsdale, New York, and by telehealth in New York, New Jersey, Connecticut, Florida and California.

The sermorelin before and after worth having is not an image. It is a baseline, a defined window, a short list of markers that were actually measured in the literature, and a clinician willing to call it off if nothing moved. Start with the health quiz or read more about sermorelin therapy.

FAQ

Frequently Asked Questions

How quickly will I see results with sermorelin?

In the trials, hormonal changes came first and outcome changes mostly did not come at all. Growth hormone rose within minutes of an injection, IGF-1 rose by week two in the 20-week trial and was drifting back toward baseline by week 16, and in that trial lean body mass changed only in men and only by 16 weeks. Nothing in the published record supports a week-by-week schedule of visible results.

What are the cons of sermorelin?

The main con is how little is known. No adult study ran past six months or enrolled more than 89 people, so cancer risk, cardiovascular outcomes and long-term safety are unstudied rather than cleared. Reported effects include injection-site reactions, flushing, headache and a temporary rise in blood lipids, and sermorelin sold today is compounded and not FDA-approved.

Will I gain weight on sermorelin?

No adult trial reported weight gain. The 20-week placebo-controlled trial reported body weight unaffected, and the six-week trial found no change in weight, body mass index, waist-to-hip ratio or DEXA-measured fat. In the University of Washington sermorelin acetate program, preliminary results from two trials showed body fat falling about 5% in men and in women not on estrogen, with a matching rise in lean mass.

Does sermorelin grow hair?

No published trial measured hair growth, so searches for sermorelin before and after hair have no evidence behind them either way. The one tissue finding in the 20-week controlled trial was increased skin thickness in both sexes, measured by caliper, which is not the same as hair growth.

What do sermorelin before and after results look like for females?

It depends on the study and on estrogen status. In a 20-week trial of ten women, skin thickness rose but lean mass, insulin sensitivity, well-being and libido improved only in the men. In the University of Washington sermorelin acetate program, preliminary results from two trials showed women not on estrogen replacement raising IGF-1 about 30% and changing body composition much as men did (in one trial alongside exercise), while women on oral estrogen saw little IGF-1 rise.

Are there real sermorelin before and after pictures?

We could not find a published trial of sermorelin or its close analogs that reports a before-and-after photograph of an adult, so the pictures circulating online are clinic marketing and personal posts rather than evidence. They have no control group, no measurement and no denominator. Protocol Health publishes none for sermorelin.

Is sermorelin before and after different when combined with TRT?

No trial has measured sermorelin and testosterone replacement together, so sermorelin and TRT before and after is an untested combination rather than a documented one. Each affects different axes, and combining two prescription therapies is a clinical decision that needs labs, monitoring and a prescriber who is tracking both.
