The Measured Timeline
What the Trials Measured, and When
A more useful TRT before and after comparison is a table of onset times. Saad and colleagues reviewed the testosterone-replacement literature for European Journal of Endocrinology in 2011, sorting effects by when each became detectable and when it stopped improving (Onset of effects of testosterone treatment and time span until maximum effects are achieved, PMID 21753068).
| Effect measured | First detectable | Maximum reported |
|---|---|---|
| Sexual interest and desire | 3 weeks | plateau at 6 weeks |
| Depressive mood | 3–6 weeks | 18–30 weeks |
| Erections and ejaculation | 3 weeks, up to 6 months in some men | 3–6 months, up to 1 year in some men |
| Lipids | 4 weeks | 6–12 months |
| Fat mass, lean mass, strength | 12–16 weeks | 6–12 months |
| Hematocrit and hemoglobin (a monitored risk) | 3 months | 9–12 months |
| PSA and prostate volume (monitored) | marginal rise | plateau at 12 months |
| Glycemic control (men with impaired glucose tolerance) | 3–12 months | not established |
| Bone mineral density | 6 months | still rising at 3 years |
On average, sexual interest and mood moved first, within three to six weeks. Fat and lean mass, the part a camera could record, took about three months to begin.
Book a TRT consultationA Controlled Trial
What One Year of Testosterone Did Against Placebo
A placebo-controlled trial puts a floor under the expectation. The Testosterone Trials randomized 790 men aged 65 and over, with symptoms and serum testosterone below 275 ng/dL, to testosterone gel or placebo gel for one year (Effects of Testosterone Treatment in Older Men, New England Journal of Medicine, 2016, PMID 26886521). Sexual activity, desire and erectile function rose significantly; mood was slightly better; vitality on a fatigue scale showed no significant benefit. The trial was too small to draw conclusions about risks.
What Photos Miss
What a Before-and-After Photograph Cannot Show
A TRT before and after photograph captures one row of that table, body composition, and none of the rest. Four problems sit behind the pictures:
- No control group. Men starting testosterone usually change training, sleep and diet the same week. A photo cannot separate hormone from habit.
- The risks are invisible. Testosterone raises hematocrit, suppresses sperm production and can raise PSA, and no camera reads any of them. The Endocrine Society guideline recommends against starting it in men planning fertility soon, or with elevated hematocrit, untreated severe obstructive sleep apnea or prostate cancer. In TRAVERSE (5,246 men, mean follow-up 33 months), major cardiovascular events occurred in 7.0% on testosterone gel and 7.3% on placebo, meeting noninferiority; atrial fibrillation, acute kidney injury and pulmonary embolism were more common on testosterone.
- Selection. Clinics publish the responders. The review's study averages for fat loss ran from 0.4 kg to 5.7 kg.
- Timing. Photographs are taken when someone is pleased with them, not on a schedule.

Our Approach
How Protocol Health Reads a TRT Timeline
A timeline means something only once the diagnosis is right. The Endocrine Society's 2018 guideline recommends diagnosing hypogonadism only in men with symptoms plus a low fasting morning total testosterone, confirmed on a repeat sample (Endocrine Society Clinical Practice Guideline, PMID 29562364). The AUA guideline uses a total testosterone below 300 ng/dL on two early-morning samples, plus symptoms.
Protocol Health prescribes testosterone therapy for men, and has a commercial interest here. Three points of status:
- Injectable testosterone esters, including testosterone cypionate, are FDA-approved drugs. Prescribing them for age-related low testosterone is an off-label use.
- Testosterone is a controlled substance — Schedule III federally, and New York Public Health Law §3306 lists it by name among the anabolic steroids in state Schedule II. Since May 21, 2025, 10 NYCRR §80.63(d)(1) bars prescribing a controlled substance before an in-person evaluation, and §80.63(e) excepts telehealth when consistent with DEA rules.
- [Enclomiphene](/treatments/enclomiphene) is unapproved — not FDA-approved for any use.
The treating clinician decides whether testosterone is appropriate, in which form and at what dose, against your labs and history. This page sets no dose. Protocol Health sees patients in person only in Scarsdale, New York, and by telehealth in New York, New Jersey, Connecticut, Florida and California. The before and after that matters is a confirmed diagnosis, effects that arrive over weeks to years, and risks monitored throughout. Start with the health quiz.
FAQ
Frequently Asked Questions
How long does TRT take to work?
It depends which effect you mean. In the Saad 2011 review, sexual interest appeared at about three weeks and plateaued at six; depressive mood improved at three to six weeks; fat and lean mass took 12 to 16 weeks.
What happens after your first testosterone injection?
Nothing the reviewed trials measured as an effect on day one. The earliest signal in the Saad 2011 review was insulin sensitivity within a week; the earliest change men report is sexual interest at about three weeks.
What changes at 3 months and 6 months on TRT?
Three months is roughly when fat mass, lean mass, strength and hematocrit became measurable; six months is when bone mineral density first changed. These are trial averages.
Does TRT change your face?
No trial measured facial appearance, so a before-and-after face comparison is uncontrolled observation rather than evidence. The Saad 2011 review does report fat mass falling from about 12 to 16 weeks.
Does testosterone affect sleep?
The Saad 2011 review did not assess sleep. The Endocrine Society's 2018 guideline recommends against starting testosterone in men with untreated severe obstructive sleep apnea.
